- Cortisone Injections for Osteoarthritis in South London | Insight Msk
À propos de nous What is Osteoarthritis ? Osteoarthritis (OA) is a common form of arthritis that primarily affects the joints. It is characterized by the gradual degeneration of the cartilage, the smooth and protective tissue that covers the ends of bones in a joint. As the cartilage wears away, bones may begin to rub against each other, leading to pain, swelling, and decreased joint mobility. For more resources on Osteoarthritis consult the reference website, Versus Arthritis . What are the key features of osteoarthritis ? Key features of osteoarthritis include: Cartilage Degeneration : In a healthy joint, cartilage provides a smooth surface that allows bones to move freely and without friction. In osteoarthritis, the cartilage breaks down and loses its ability to cushion the bones. Bone Changes : Over time, the bones in the joint may undergo changes, such as the formation of bone spurs (osteophytes) in response to the stress caused by the loss of cartilage. Joint Inflammation : While osteoarthritis is not primarily an inflammatory condition like rheumatoid arthritis, inflammation may occur as a secondary response to cartilage damage. Symptoms : Common symptoms of osteoarthritis include joint pain, stiffness, and reduced range of motion. These symptoms often worsen with activity and improve with rest. Weight-bearing joints such as the knees, hips, and spine are commonly affected, but osteoarthritis can occur in any joint. Which factors can contribute to the development of osteoarthritis ? Several factors contribute to the development of osteoarthritis, including: Aging : The risk of osteoarthritis increases with age. Joint Overuse or Injury : Previous joint injuries or repetitive stress on a joint can contribute to the development of osteoarthritis. Genetics : There is a genetic component to osteoarthritis, and it may run in families. Obesity : Excess body weight can increase the risk of osteoarthritis, particularly in weight-bearing joints. What is the best treatment for osteoarthritis? Management of osteoarthritis involves a combination of lifestyle modifications, pain management, and, in some cases, surgical interventions. Treatment options include: Exercise: Low-impact exercises to improve joint flexibility and strengthen the muscles around the affected joint. Weight Management: Maintaining a healthy weight can reduce stress on weight-bearing joints. Medications: Pain relievers, anti-inflammatory medications, and in some cases, joint injections. Physical Therapy: Targeted exercises to improve joint function and reduce pain. Surgery: In severe cases, joint replacement surgery may be considered. It's important for individuals with symptoms of osteoarthritis to consult with a healthcare professional for an accurate diagnosis and appropriate management plan. How can an ultrasound guided injection help? Injections can be a part of the treatment plan for osteoarthritis (OA) and may be recommended to help manage pain and inflammation associated with the condition. Several types of injections are commonly used in the management of osteoarthritis, and they can provide varying degrees of relief. Here are some common types of injections we offer at Insight MSK : Corticosteroid Injections : These injections deliver corticosteroid medications directly into the affected joint. Corticosteroids are powerful anti-inflammatory drugs that can help reduce inflammation and alleviate pain. The effects of corticosteroid injections are often temporary, providing relief for a few weeks to several months. These injections are commonly used for joints such as the knee, hip, or shoulder. Hyaluronic Acid Injections : Also known as viscosupplementation, hyaluronic acid injections involve injecting a gel-like substance into the joint to improve lubrication and cushioning. Hyaluronic acid is a component of the synovial fluid, which naturally lubricates the joints. This type of injection is often used in the knee and may provide relief for several months. It's important to note that the choice of injection depends on factors such as the specific joint involved, the severity of symptoms, and the patient's overall health. Injections are often considered when other conservative measures, such as physical therapy and medications, have not provided sufficient relief. While injections can be beneficial for managing osteoarthritis symptoms, they are not a cure for the underlying condition. Additionally, the decision to pursue injections should be made in consultation with a healthcare professional who can assess the individual's condition and recommend the most appropriate treatment plan. Book an appointment with our expert Maxence Legout who is a specialist in the field and been seen now. - Frozen Shoulder Injections in South London | Insight Msk
À propos de nous What is a Frozen shoulder ? Frozen shoulder, also known as adhesive capsulitis, is a condition characterized by pain, stiffness, and restricted range of motion in the shoulder joint. It typically develops gradually over time and can progress through distinct stages. Key features of frozen shoulder include: Pain : Individuals with frozen shoulder often experience pain in the shoulder, which may be more pronounced at night. The pain is usually located deep in the shoulder joint. Stiffness : Stiffness is a hallmark of frozen shoulder. The shoulder gradually becomes more difficult to move, leading to a progressive loss of range of motion. Restricted Range of Motion : The loss of shoulder mobility affects both active and passive movements. Activities such as reaching overhead, reaching behind the back, or even basic activities like combing hair may become challenging. Stages : Frozen shoulder typically progresses through three stages: Pain (Freezing) Stage : The initial stage is characterized by the gradual onset of pain, especially with movement. As pain increases, shoulder mobility starts to decline. Frozen Stage : Stiffness becomes more prominent, and range of motion continues to decrease. Pain may persist but often becomes less intense. Thawing Stage : During this stage, there is a gradual improvement in shoulder mobility. Range of motion begins to return, and pain subsides. What causes a Frozen shoulder ? The exact cause of frozen shoulder is not always clear, but certain factors may contribute, including inflammation, thickening, and tightening of the joint capsule surrounding the shoulder joint. Certain conditions, such as diabetes, cardiovascular disease, and prolonged immobility, may increase the risk of developing frozen shoulder. What is the best treatment for the Frozen shoulder ? Treatment for frozen shoulder may include: Pain Management : Pain relief measures, such as over-the-counter pain medications or prescription medications, may be recommended. Physical Therapy : Gentle stretching and range-of-motion exercises prescribed by a physical therapist can help improve shoulder mobility. Heat and Ice : Applying heat or ice to the shoulder may provide relief from pain and inflammation. Joint Distension : In some cases, a healthcare professional may use a procedure called joint distension, which involves injecting sterile water into the joint capsule to stretch it. Corticosteroid Injections : Injections of corticosteroids into the joint may be used to reduce inflammation. Home Exercises : Performing prescribed exercises at home as part of a regular routine. While the symptoms of frozen shoulder can be debilitating, the condition is usually self-limiting, and most individuals will regain shoulder function over time with appropriate treatment. In severe cases or when conservative measures are not effective, surgical intervention may be considered. We do not actually inject cortisone as per say as it is short acting and less potent, but we use medications of the same family such as Triamcinolone Acetonide, Methylprednisolone or Dexamethasone which last longer and are more effective to manage inflammation, decrease pain, swelling and stiffness. We usually combine these injections with anaesthetic drugs to reduce the discomfort of the injection and limit pain afterward. These are very safe and effective treatment which can be used for a high range of conditions. Viscosupplementation / Hyaluronic Acid Injections Hyaluronic acid is one component of the fluid found into your joints (synovial fluid). They produce immediate lubrication and improve shock absorption into your joints. This treatment has been proven safe and effective for chronic moderate osteoarthritis. We typically recommend these injections for knees and hips. We use Ostenil® Plus. Tendon Fenestration (Dry Needling) Also called Percutaneous Tenotomy, fenestration is performed on chronic tendon disorders (tendinopathies) which have not improved after at least 6 months of conservative management. During the procedure, we perform several small incisions with a small needle in the affected tendon under local anaesthesia and ultrasound guidance. These incisions can stimulate blood flow to the area and promote the body's natural healing response. The process also triggers an inflammatory reaction, which helps in the repair of the tendon. This is followed by an intensive rehabilitation period. We typically recommend this treatment for non improving tennis or golfer's elbow. Shoulder hydrodistension/hydrodilatation This technique is used to treat capsulitis (frozen shoulder) when other treatments have failed. After numbing the shoulder with an anaesthetic agent, sterile saline is injected with a corticosteroid to attempt to stretch the joint and help to recover some mobility. Tendon Barbotage We use this method to treat resistant calcific tendinopathy on the rotator cuff. After an ultrasound to identify the calcification, we inject an anaesthetic agents to numb the region. We then break the calcification with a needle under ultrasound guidance and we aspirate the calcium deposit when possible. Research suggests up to 70% success rate with this procedure.
